Information Collected or Received from You
Our primary goals in collecting information are to provide and improve our Services, to administer your use of the Services (including your Account, if you are an Account holder), and to enable you to enjoy and easily navigate our Services. As a reminder, we do not sell information, and we do not use your sample results for marketing or personalized advertising without your explicit consent.
Here are the types of Information we collect:
• Registration Information: Information you provide during account registration or when purchasing the Services. Such information includes name, user ID, password, date of birth, billing address, shipping address, payment information, account authentication information, or contact information (“PII”).
• Sample Information: information regarding your sample, such as stool sample, that your submit for processing to be analyzed to provide you with microbiome results, laboratory values or other data provided though our Services.
• Self-Reported Information: information you provide to GutCheckTM regarding your disease conditions, health-related information, ethnicity, life-style information, or anything else you provide to us within our Services.
• Web-Behaviour Information: information on how you use our Services or about the way your devices use our Services is collected though log files, cookies, web beacons, and similar technologies (e.g., Google Analytics).
“Cookies” are small text files that are placed on your device by a Web server when you access our Services. We may use both session Cookies and persistent Cookies to identify that you’ve logged in to the Services and to tell us how and when you interact with our Services. We may also use Cookies to monitor aggregate usage and web traffic routing on our Services and to customize and improve our Services. Unlike Cookies, which are stored on the user’s device, Web Beacons is typically embedded invisibly on web pages (or in an e-mail).

GutCheckTM Research analyses are conducted with information that has been de-identified of any personal identifiable information.
By participating in our Services, you consent to:
• Your de-identified Sample Information are/or Self-Reported Information may be used for Research.
• We may use de-identified individual-level Sample Information and Self-Reported Information internally at GutCheckTM for research purposes.
• We may share summaries of research results, which do not identify any individual, with qualified research collaborators and in scientific publications.
• We will not share individual-level Personal Information without your explicit consent.
